Diabetic Ketoacidosis – Your Health Questions

David asks…
Can septic abortion precipitate diabetic ketoacidosis ?
CAME ACROSS A PATIENT WITH SEPTIC ABORTION AND DIABETIC KETOACIDOSIS.REVIEW OF CURRENT LITERATURE DOES NOT LIST SEPTIC ABORTION AS A KNOWN PRECIPITATING FACTOR.

Linda answers:
Any sepsis would make managing Type 1 diabetes difficult. If the patient did not have guidance, then they could easily slip into DKA.

Helen asks…
Why does Diabetic Ketoacidosis cause stroke?
why is stroke a complication of DKA?

Linda answers:
“Although possibly under-appreciated, up to 10% of cases of intracerebral complications associated with an episode of DKA, and/or its treatment, in children and youth are due to hemorrhage or ischemic brain infarction. Systemic inflammation is present in DKA, with resultant vascular endothelial perturbation that may result in coagulopathy and increased hemorrhagic risk.” From a document called “DIABETIC KETOACIDOSIS-ASSOCIATED STROKE IN CHILDREN AND YOUTH”
What it means is that a combination of inflamed blood vessels and blood clots, both caused by DKA combine to create a high risk of stroke.

Ruth asks…
Diabetic Ketoacidosis prehospital care?
How would an EMT, not a paramedic, deal with a person in diabetic ketoacidosis? What kind of treatment would you use and what would their vital signs typically be? I am looking for a specific range of heart rate, blood pressure, etc. Not just tachycardia, tachypnea, etc.
Have you ever come across a patient with DKA? What did they look like, how did they act? Any other info you have would be great.
Thanks!

Linda answers:
Holly is right on
